Tyrone Regional Health Network (TRHN) has several primary care physician offices that are departments of Tyrone Hospital. These include the Tyrone Rural Health Center located on the TRHN campus, Pinecroft Medical Center, the office of Jerome DeJulia, M.D. and Fawzia Haque, M.D., Michael Halter, D.O., Bobbie Oshall, CRNP and Rachel Mock, PA-C located in the Pinecroft area of Altoona and Houtzdale Rural Health Center located in Houtzdale.

We provide primary care services for adults of all ages and children age 12 and older. We provide care for a wide variety of non-emergency medical problems. We also provide services for routine health needs including annual physicals,health screenings, immunizations and other services. We can also help with chronic health conditions like high blood pressure, diabetes and other conditions. we can help you with preventative care as well, to keep you healthy and lower your risk of disease and illness. if you need any type of specialized care, we will refer you to an appropriate specialist to be sure your total health picture is properly addressed.
Additionally, we provide Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses. The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.

Chronic Care Management (CCM) Service
Pinecroft Medical Center provides Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses. The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health. Those who participate in the Chronic Care Management Service access a healthcare professional in between medical appointments and as needed for help to keep track of your medical history, medications, and all the doctors you see. You’ll also receive a comprehensive care plan that outlines your treatment plan and goals.
