Tyrone Regional Health Network (TRHN) has several primary care physician offices that are departments of Tyrone Hospital. These include the Tyrone Rural Health Center located on the TRHN campus, Pinecroft Medical Center, the office of Jerome DeJulia, M.D. and Fawzia Haque, M.D., Bobbie Oshall, CRNP and Rachel Mock, PA-C located in the Pinecroft area of Altoona and Houtzdale Rural Health Center located in Houtzdale.

Our Services

We provide primary care services for adults of all ages and children age 12 and older. We provide care for a wide variety of non-emergency medical problems. We also provide services for routine health needs including annual physicals,health screenings, immunizations and other services. We can also help with chronic health conditions like high blood pressure, diabetes and other conditions. we can help you with preventative care  as well, to keep you healthy and lower your risk of disease and illness. if you need any type of specialized care, we will refer you to an appropriate specialist to be sure your total health picture is properly addressed.

Additionally, we provide Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.

Chronic Care Management (CCM) Service

Pinecroft Medical Center provides Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.  Those who participate in the Chronic Care Management Service access a healthcare professional in between medical appointments and as needed for help to keep track of your medical history, medications, and all the doctors you see. You’ll also receive a comprehensive care plan that outlines your treatment plan and goals.

Our Team

Our Chronic Care Team is led by the Chronic Care Manager Rebecca Swintek, Pharm.D.  She and her Team oversee all Annual Wellness Visits and Chronic Care Management Service patients and works directly with the doctors of Tyrone Regional Health Network to give you the best quality of care. The Chronic Care Management team is a vital part of the organization and works to keep patients first.

Annual Wellness Visit

  • For patients who have had Medicare for more than 12 months. Medicare allows one Annual Wellness Visit every 365 days.
  • FREE – there is no cost for patients with Medicare
  • Not a physical exam
  • Opportunities to have face-to-face time with the Chronic Care Team to go over your individual health history, set up preventative screenings and services, and make up a health plan
  • Helps regular doctor visits run more smoothly and efficiently.  Your doctor can look at your custom-made plan and know your most important health risks and concerns.  This gives you more time during regular doctor visits to talk about new health issues.
