Restrictions and New Procedures Implemented Due to COVID-19 (updated 7-9-2020)

Houtzdale Rural Health Center has implemented additional procedures for patient protection and safety as follows:

When patients call to make an appointment staff will ask if you believe you have Coronavirus (COVID-19) or have been exposed to the virus, have traveled to a high risk area, or if you have any symptoms like cough, fever or shortness of breath.

For patients who need to visit the office for a face-to-face appointment, chairs are arranged in the general waiting areas to promote social distancing

Patients with signs or symptoms of cough, shortness of breath, fever, positive exposure history or positive travel history will be seen in the parking lot. Patients will be given a dedicated cell phone number to call upon arrival and nursing staff and providers will see patients in the parking lot in order to minimize exposure.

To limit the number of people who enter the offices, patients are asked to please come to their appointment alone, if that is possible. One person may accompany a patient if the patient is a minor or if the patient has a caregiver who is essential for communicating with the patient’s provider. Anyone who accompanies a patient but does not need to remain with the patient during their appointment will be asked to wait in their vehicle.

Telehealth visits continue to be available as an option for  patients.

Community members are reminded to continue taking steps to protect themselves from illness as well as spreading illness to others. Wash your hands often. Cover coughs and sneezes with a tissue that you discard or cough and sneeze into your elbow. Don’t touch your face. Clean surfaces often. Stay home when you are sick and avoid sick people. Social distancing is also recommended with people advised to limit their contact in public and in crowds to reduce exposure and the spread of illness.

People should also be aware of the signs of COVID-19.

According to the Centers for Disease Control (CDC), people with COVID-19 have had a wide range of symptoms reported – ranging from mild symptoms to severe illness.

Symptoms may appear 2-14 days after exposure to the virus. People with these symptoms or combinations of symptoms may have COVID-19:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

      Children have similar symptoms to adults and generally have mild illness.

      This list is not all inclusive. Please consult your medical provider for any other symptoms that are severe or concerning to you.

      About Us

      The Houtzdale Rural Health Center is a primary care physician practice, state certified rural health clinic (RHC), in Houtzdale, Pennsylvania. We provide services to adults age 18 years of age and older.

      Our clinic provides primary healthcare to the residents of  Houtzdale, PA and surrounding communities.   We provide services to people regardless of their age, race, creed, sex, national origin, or ability to pay.

      Our facility is handicap accessible and meets the requirements of the Americans with Disabilities Act (ADA).

      The Houtzdale Rural Health Center is a 501(c)(3) not-for-profit corporation.


      Houtzdale Rural Health Center practitioners address daily patient care needs by diagnosing and treating illnesses and injuries as well as providing initial preventive physical examinations and employer/institution mandated examinations and vaccinations.  Blood draws for patients’ lab testing is also provided.

      We provide Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.

      Additionally, we provide Medicare Annual Wellness Visits to our Medicare patients.

      Telehealth Appointments

      Houtzdale Rural Health Center offers patient appointments using telehealth services. The telehealth services are available to patients as an option and are in addition to the traditional in office appointments offered.

      Telehealth is the use of technology such as videos and phone calls to connect patients with their healthcare providers.   It allows patients to connect with their health provider for assessment, treatment, monitoring or education from the comfort of their own home. This is accomplished using technology such as a cell phone, laptop computer, desktop computer, ipad, a streaming service or other device.

      Staff at our office communicates with patients in advance of their Telehealth visit to provide access instructions.

      Telehealth visits have a copay just like normal offices visits.

      Most insurances pay for Telehealth visits just as they do for in office appointments.

      For patients who do not use electronic devices, they can talk to family members or friends to see if someone they know has a way to help them make a video call.

      If through the video call it is determined that a patient needs to come to the office in person or needs to see a specialist, the doctor’s office will make a plan for that with the patient.

      The communication between doctor and patient through the video call is secure to insure patient privacy.

      Houtzdale Rural Health Center is open Mondays – Friday from 9:00 AM – 5:00 PM. Patients are seen by appointment.

      Sliding Fee Discount

      The Houtzdale Rural Health Center is a state certified rural health clinic.  We are able to offer a discount on some service’s based on a family’s income and size.

      Determining Eligibility for the Sliding Fee Discount
      Eligibility for the sliding fee discount is determined based on a family’s income and size.  The Houtzdale Rural Health Center Sliding Fee Discount Application must be completed and submitted. The Sliding Fee Discount Scale is then used to determine your eligibility.

      Your family discount will be determined on a yearly basis.

      It should be noted that there is a minimum charge amount for some procedures, labs and medications.

      Sliding fee payment is due and payable at the time of service. To maintain the sliding fee discount, fees must be paid promptly.  If you are unable to make payment at time of service, please speak to the receptionist to make other arrangements.  Failure to do so may result in loss of discount.

      Chronic Care Management Services and Annual Wellness Visit

      Chronic Care Management Services

      The Tyrone Rural Health Center provides Chronic Care Management (CCM)Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.  Those who participate in the Chronic Care Management Service access a healthcare professional in between medical appointments and as needed for help to keep track of your medical history, medications, and all the doctors you see. You’ll also receive a comprehensive care plan that outlines your treatment plan and goals.

      If you are a patient at the Tyrone Rural Health Center, have Medicare and live with two or more chronic conditions, you may be eligible for Chronic Care Management Services. For more information or to see if you are eligible, call the CCM Team directly at (814) 682-1850.

      Annual Wellness Visit

      • For patients who have had Medicare for more than 12 months. Medicare allows one Annual Wellness Visit every 365 days.
      • FREE – there is no cost for patients with Medicare
      • Not a physical exam
      • Opportunities to have face-to-face time with the Chronic Care Team to go over your individual health history, set up preventative screenings and services, and make up a health plan
      • Helps regular doctor visits run more smoothly and efficiently.  Your doctor can look at your custom-made plan and know your most important health risks and concerns.  This gives you more time during regular doctor visits to talk about new health issues.
