Your Feedback is Important to Us

As a patient of Tyrone Regional Health Network, you have the right to receive high quality care and services. We continuously strive to improve our performance while assuring our patients and their families/friends receive the services and care they deserve.

If you feel a staff member has gone above and beyond to make your stay wonderful, we would love to hear about it. In the event our services do not meet your expectations, we encourage you to bring your concerns to the attention of a staff member. If the staff member is unable to resolve the issue immediately, it is your right to register a grievance. A grievance is a formal process to address a concern or issue about the quality of the care and/or services provided to a patient.

You may submit a grievance or any other type of feedback you may wish to share via our online feedback back form. You can access the form by clicking the link below.

Please provide the following information when registering a grievance:

  • Patient name, address, and phone number
  • If you are not the patient, your name and relationship to the patient.
  • Federal regulations which protect patient confidentiality require patient authorization before we can discuss the patients’ treatment with you.
  • The room number or department where services were received.
  • Date of hospitalization or services
  • Background of the situation such as time, place, and people involved
  • A description of the situation or problem
  • The best time of day and phone number to contact you for follow up

We will address your concern as promptly as possible. At a minimum we will provide a written response to you within seven working days from the day that we received your grievance.

You may also call or write the Pennsylvania Department of Health or your insurance company to place a grievance about the quality of care received.
