About Us

The Tyrone Rural Health Center is a primary care physician practice, state certified rural health clinic (RHC), in Tyrone, Pennsylvania.

Our clinic provides primary healthcare to the residents of Tyrone, PA and surrounding communities.   We provide services to people regardless of their age, race, creed, sex, national origin, or ability to pay.

The Tyrone Rural Health Center is conveniently located on the Tyrone Hospital campus at 221 Hospital Drive, Suite 6, Tyrone, PA. Our facility is handicap accessible and meets the requirements of the Americans with Disabilities Act (ADA).

The Tyrone Rural Health Center is a 501(c)(3) not-for-profit corporation.


Tyrone Rural Health Center practitioners address daily patient care needs by diagnosing and treating illnesses and injuries as well as providing initial preventive physical examinations and employer/institution mandated examinations.

We provide Chronic Care Management Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.

Additionally, we provide Medicare Annual Wellness Visits to our Medicare patients.

We are also a provider of the HealthyWoman Program, a program of the Pennsylvania Department of Health that provides free cancer screening to eligible women age 21 and over.

Monday, Tuesday and Friday: 8:00 am to 5:00 pm

Wednesday and Thursday 8:00 am to 6:00 pm

Sliding Fee Discount

The Tyrone Rural Health Center is a state certified rural health clinic.  We are able to offer a discount on some service’s based on a family’s income and size.

Determining Eligibility for the Sliding Fee Discount
Eligibility for the sliding fee discount is determined based on a family’s income and size.  The Tyrone Rural Health Center Sliding Fee Discount Application must be completed and submitted. The Sliding Fee Discount Scale is then used to determine your eligibility.

Your family discount will be determined on a yearly basis.

It should be noted that there is a minimum charge amount for some procedures, labs and medications.

To qualify for the sliding fee, you must complete and submit the Tyrone Rural Health Center Sliding Fee Discount Application.

Sliding fee payment is due and payable at the time of service. To maintain the sliding fee discount, fees must be paid promptly.  If you are unable to make payment at time of service, please speak to the receptionist to make other arrangements.  Failure to do so may result in loss of discount.

Chronic Care Management Services and Annual Wellness Visit

Chronic Care Management Services

The Tyrone Rural Health Center provides Chronic Care Management (CCM)Services to help our Medicare patients who have two or more chronic conditions or illnesses.  The service provides personal attention from a healthcare professional to help you better manage your care and help you make the best choices for your health.  Those who participate in the Chronic Care Management Service access a healthcare professional in between medical appointments and as needed for help to keep track of your medical history, medications, and all the doctors you see. You’ll also receive a comprehensive care plan that outlines your treatment plan and goals.

If you are a patient at the Tyrone Rural Health Center, have Medicare and live with two or more chronic conditions, you may be eligible for Chronic Care Management Services. For more information or to see if you are eligible, call the CCM Team directly at (814) 682-1850.

Annual Wellness Visit

  • For patients who have had Medicare for more than 12 months. Medicare allows one Annual Wellness Visit every 365 days.
  • FREE – there is no cost for patients with Medicare
  • Not a physical exam
  • Opportunities to have face-to-face time with the Chronic Care Team to go over your individual health history, set up preventative screenings and services, and make up a health plan
  • Helps regular doctor visits run more smoothly and efficiently.  Your doctor can look at your custom-made plan and know your most important health risks and concerns.  This gives you more time during regular doctor visits to talk about new health issues.

HealthyWoman Program

The Tyrone Rural Health Center  is a provider in the Pennsylvania Department of Health’s HealthyWoman Program. The program provides free cancer screening to eligible women age 21 and over. This gives women in Blair, Clearfield, Centre, Cambria and Huntingdon counties easier access to essential women’s health services.

Services include clinical breast examination, mammogram, pelvic examination, Pap smear and follow-up diagnostic tests for an abnormal screening result.

Eligibility for the program can be determined in a matter of minutes. The enrollment process is also easy. It involves a few questions and completion of short application. Women age 21 and over may be eligible for breast screening and diagnostics and for cervical screening and diagnostics. Younger women may be eligible if they have symptoms.  Additionally, a woman must be a resident Pennsylvania, be uninsured or underinsured and have a gross household income that is at or below 250% of the federal poverty annual guideline.

While breast cancer screening typically applies to women age 40 and over, younger women who have been assessed by their health care provider to be at high risk for breast cancer may be eligible for breast cancer screening through the HealthyWoman Program.  High risk includes genetic mutations, family history and some other factors.  Women are advised to talk to their health care provider about their breast cancer risk.

Transgender individuals may also be eligible.

Funding for the HealthyWoman Program is provided through a grant the from the Centers for Disease Control and Prevention (CDC).
